Membership Application and Renewal
Thank you for your interest in our Association. Please click the file link to the left to open the fillable form in you browser. Dues are $50 for the 2012 year. Please complete the form and mail to the address on the form with your payment of $50. We are not able to process credit card payments. Payent by check or money order only. Please note that membership is based from January 1st. to December 31st of each calander year. Membership fees expire on December 31, of the same year, regardless of when the fee(s) were paid.

Annual Scholarship Application
Each year DFSA awards multiple scholarships to quailifying Mortuary Science Students (see criteria below). Sponsoring the scholarships are Chesapeake Burial Vault Comany and Delmarva Funeral Service Association. Please click on the file link to the left to download the application.
If you have any questions, please email the association office at info@delmarvafuneralservice.com or call 410-205-2430 with any questions. Applications are due no later than November 1 of each year, and will be awarded at the annual holiday party.

Criteria for application
- Must be a mortuary science student from Maryland, Delaware or Virginia.
- Should have at least a 3.0 grade point average to receive and reapply for the scholarship (proof must be shown). If a first year college student, should show proof of at least a 3.0 GPA from high school.
- A letter of recommendation from the mortuary science advisor is needed from applicant’s mortuary college.
- Applicant must be enrolled in at least two classes per semester. Proof of this may be required.
- A review board made up of the board of directors of the Delmarva Funeral Service Association and an associate of Chesapeake Burial Vault will review applications.
- Scholarships of $1000.00 will be awarded annually.
- Applications must be submitted yearly, old applications will not be considered for review.
- Scholarships that are awarded will be paid directly form the association to the mortuary college.
